Scope Of Work *

Includes the removal and disposal of any existing backflow prevention assembly, if one exists, and the installation of a new dual check backflow prevention assembly. The installation shall be performed to meet the standards set forth by the City of Apopka. All repair and/or installation work of testable devices includes the certification test(s) and the submission of the test report(s) to the authority having jurisdiction.

Exclusions *

The following items are specifically excluded from this proposal: Responsibility for customer’s and/or resident’s plumbing system of any kind including but not limited to pipes, plumbing fixtures and appliances. Responsibility for any drop in pressure due to backflow preventer installation or maintenance . Responsibility for any leaking connection or piping to the backflow valve unless specifically included in the contract. Notification of water interruption to customers and/or residents ahead of time. Notification to customers and/or residents of PVC glue and primer odor in the water after plumbing system has been worked on. Boil water notices. Painting or preparation for painting. Electrical wiring, conduit, devices, and equipment. Alarm and/or monitoring system work. Hiding or shielding of any exposed piping if necessary. Insulation of any pipe. Relocation of existing piping due to conflicts with others. Any underground backflows, excavation or backfill. Utility water connection, impact fees and metering fees.

General Terms and Conditions *

This proposal is based on normal business hours, 7am-5pm, Monday-Friday (excluding all holidays) 2 hours minimum not including drive time. Ace Flow Control will not provide overtime required due to schedule revisions, work stoppage, delays caused by others or circumstances beyond its control. No material will be ordered, or design/engineering performed until Ace Flow Control is in receipt of an approved agreement or purchase order. Ace Flow Control agrees to no back charges without adequate written notice, ample time to rectify any associated condition, and prior acceptance and agreement of Ace Flow Control. Invoicing is either due at time of completion or NET 15 from date of invoice. Proposals in excess of $2,000 may require 50% down payment. Any alteration or deviation from the above involving additional cost will only be performed following written acceptance by Ace Flow Control and may result in additional costs. Ace Flow Control may withdraw this proposal if not accepted within 30 days from the date shown on page 1. This proposal is based on the drawings and specifications provided at time of bid. No additional costs have been included for unseen RFI’s, addendums, or additional devices not shown. This Proposal Does not include any provisions to bring existing systems to current codes unless specifically included in the scope of work listed above. This proposal includes a one (1) year limited warrantee on all new equipment. Exclusions; fire, theft, vandalism, acts of god, lightning strikes, surges/spikes and/or tampering/work by unauthorized individuals.
